Job summary

Crossbridge Hospice seeks a Sales Clinical Liaison (RN) to advocate for patients through referral and admission, ensuring appropriate care levels. This clinician-reliant role emphasizes relationship-building with referral sources and community partners, while educating families about hospice options and enrollment steps.

The position requires an active RN license (state of practice) and ADN/BSN. Experience in case management is preferred; opportunities for growth and a mission-driven team

Qualifications

• The candidate must be a Registered Nurse with an active, unrestricted license in the state of practice.
• Associate’s or Bachelor’s degree in nursing (ADN or BSN) required.
• Minimum two years of clinical nursing experience, including case management, preferred.

Responsibilities

• Act as patient advocate during referral and admission processes.
• Grow organization by developing referral relationships and community presence.
• Educate patients and families about hospice services and enrollment steps.
• Coordinate with referral sources and clinical teams for timely follow-ups.
