Crossbridge Hospice is seeking a Sales Clinical Liaison to advocate for patients through referral and admission, requiring RN licensure and clinical credibility. You will build referral relationships, educate families about hospice options, and coordinate with clinicians to assure smooth enrollment.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Associate’s or Bachelor’s degree in nursing (ADN/BSN).
• Strong communication and relationship-building abilities.

Responsibilities

• Act as patient advocate through referral and admission processes.
• Develop referral relationships and represent the company in the community.
• Educate patients and families about hospice services and enrollment.
• Coordinate with healthcare providers and ensure timely follow-ups.
• Support market leadership and position the organization as a trusted leader.
