Crossbridge Hospice, LLC is seeking a Sales Clinical Liaison (RN) to advocate for patients throughout referral and admission, guiding them to appropriate care levels. The role requires an RN license and clinical insight to educate families about hospice services.

You will build referral relationships, collaborate with clinical teams, and drive community outreach to promote compassionate, patient-centered hospice care.

Qualifications

• Associate’s degree in nursing (ADN) or Bachelor of Science in Nursing (BSN).
• Active, unrestricted Registered Nurse (RN) license in the state of practice.
• Strong communication, relationship-building and organizational skills.

Responsabilités

• Proactively grow the organization by identifying new opportunities, building strong referral relationships, and representing the company in the community.
• Act as a trusted, compassionate resource, educating patients and families about hospice services and guiding them through enrollment.
• Strengthen partnerships with healthcare providers, ensure smooth communication between referral sources and clinical teams, and manage timely follow-ups and documentation.
• Drive awareness and growth by educating stakeholders, analyzing referral trends, staying informed on market dynamics, and positioning the organization as a trusted leader in the community.
