Job summary

Interim HealthCare Inc. in Lima, OH is seeking a Hospice Client Care Coordinator (CCC) to support a full range of patient services, bringing comfort and dignity to our clients. You will manage enrollment assessments, coordinate with RN and pharmacy, document appointments, and communicate with families by phone and computer.

A valid driver’s license and prior experience in senior care or hospice are preferred. Competitive pay and benefits are offered.

• One year of related business experience.
• Experience in working in the senior field or hospice preferred.

Responsibilities

• Facilitate new client enrollment within initial and subsequent screening assessments and further maintain client’s healthcare record.
• Identify resources to address client’s health, daily living, and safety needs.
• Perform administrative tasks such as scheduling, logging, documenting all appointments and services.
• Communicate to clients through telephone and computer.
• Coordinate with RN and pharmacy medication set-ups and reminders.
• Be a family liaison and coordinate with the family, designees, and providers.
