Job summary

Arc Hospice & Palliative Care Internal Careers is seeking a Chaplain to provide spiritual support to patients, families, and the interdisciplinary team. You will conduct spiritual assessments, develop care plans, offer counseling, and coordinate rituals or sacraments as requested by patients or families.

You will participate in IDG meetings, document services in the EMR, and assist with advance care planning and bereavement support.

Qualifications

• Master’s degree in divinity, theology, pastoral counseling, or related field.
• CPE completion required; two units preferred or board certification.
• Ordination/endorsement by a recognized faith group.
• Hospice/hospital or community health care experience preferred.
• Ability to serve diverse populations with respect to faith, culture, identity, and beliefs.
• Knowledge of Medicare hospice regulations and Conditions of Participation related to spiritual care.
• Valid driver’s license and reliable transportation for patient visits.
• Excellent interpersonal, documentation, and communication skills.

Responsibilities

• Conduct spiritual assessments and develop individualized spiritual care plans for patients and families.
• Provide ongoing spiritual and emotional support aligned with patient beliefs and preferences.
• Support patients and families of all faiths or belief systems, including non-religious or secular individuals.
• Offer prayer, sacraments, rituals, or spiritual rites as appropriate.
• Participate in interdisciplinary group meetings and contribute to holistic care planning.
• Document spiritual care services and care plan updates in the EMR system.
• Assist with advance care planning, legacy work, or life review on request.
• Coordinate with community clergy to ensure continuity and cultural relevance of care.
• Assist with bereavement care and family support before and after death.
