Job summary

Heart to Heart Hospice is seeking a Chaplain PRN to provide compassionate spiritual care to patients and families facing life-limiting illness. You will assess spiritual needs, offer counseling, and collaborate with the IDG to integrate care plans.

Minimum one year in healthcare or death-and-dying settings is expected, with comfort in interfaith pastoral work and strong English communication. A valid driver’s license and EMR experience are preferred.

Qualifications

Advanced pastoral knowledge via formal education, CPE, board certification, or ordained ministry.
Minimum one year in healthcare settings or death and dying experiences.
Ability to work across faith traditions and provide spiritual care without imposing beliefs.
Current state driver's license and reliable transportation.

Responsibilities

Independently manage an assigned caseload of hospice patients, scheduling visits and spiritual approach.
Conduct individualized spiritual assessments for patients and families.
Provide pastoral counseling and crisis intervention in real time.
Participate in Interdisciplinary Group (IDG) with care team.
Manage bereavement relationships with families after patient death.
Collaborate with clinical team to address spiritual and ethical issues.
