Job summary

Heart’n Soul Hospice seeks an experienced Chaplain to provide compassionate spiritual care to hospice patients and families in a multidisciplinary team. You will assess spiritual needs, offer counseling, and collaborate with nursing, social work, and clinical leadership to plan care.

Requirements include advanced pastoral knowledge through an accredited pathway, healthcare experience, and ability to work across faith traditions with professionalism.

Qualifications

• Minimum one year of experience in a healthcare setting or with death and dying.
• Demonstrates comfort working in an interfaith and multi-belief setting; ability to provide spiritual care without imposing personal beliefs.
• Proficiency with computer systems including EMR software; experience with FireNote preferred.
• Strong written and verbal English communication skills.
• Demonstrated ability to manage an independent caseload with minimal supervision.

Responsibilities

• Independently manage an assigned caseload of hospice patients, determining visit frequency, scheduling, and pastoral approach.
• Conduct individualized spiritual assessments for patients and families to identify spiritual needs and resources.
• Provide professional pastoral counseling, crisis intervention, and emotional support in real time.
• Participate in Interdisciplinary Group (IDG) meetings, presenting spiritual assessments and care plans.
• Manage bereavement support with families after patient death, tailoring duration and content.
• Collaborate with clinical team to resolve spiritual and ethical issues in patient care.
• Maintain compliance with regulations and internal Heart to Heart policies.
• Perform other duties as assigned.
