Job summary

Heart to Heart Hospice in San Antonio, TX is seeking a Chaplain to provide compassionate spiritual care to patients with life-limiting illnesses and their families within an interdisciplinary team.

The Chaplain will assess spiritual needs across faith traditions, offer crisis counseling, bereavement support, and work with nurses, social workers, and clinicians to create individualized care plans while upholding regulations and agency policies.

Qualifications

• Formal theological education as listed (BDiv/MDiv or equivalent).
• CPE completed or in progress (ACPE-accredited).
• Board Certification or equivalent credential preferred.
• Or ordained ministry with 3+ years in grief/bereavement hospice context.

Responsibilities

• Independently manage an assigned caseload of hospice patients with tailored spiritual plans.
• Conduct individualized spiritual assessments across faith traditions for patients and families.
• Provide pastoral counseling and crisis intervention in real time.
• Present spiritual assessments and care plans at the Interdisciplinary Group (IDG) meetings.
• Maintain bereavement support with families after patient death, adjusting contact as needed.
• Collaborate with the clinical team to resolve spiritual and ethical issues in patient care.
• Ensure compliance with federal/state regulations and internal policies.
