Job summary

Compassus is seeking a Chaplain to provide hospice-focused spiritual care to patients and families. The role supports the patient’s beliefs, collaborates with the interdisciplinary team, and coordinates with community clergy as needed.

The Chaplain will ensure care plans reflect spiritual needs, assist in bereavement support, and maintain compassionate, nondiscriminatory service in a challenging healthcare environment.

Qualifications

• High school diploma or GED required; bachelor's preferred where state regulation allows.
• Minimum 1 year of experience in spiritual/pastoral support.
• Experience with loss, grief, bereavement or trauma preferred.
• CPE unit completion preferred where required by state regulation.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent.
• Submits documentation to facilities after patient visits per policy.
• Articulates regulatory framework governing spiritual care at federal/state levels.
• Participates as a member of the IDT and helps develop the plan of care.
• Provides spiritual care to patients and families; coordinates with community clergy as requested.
• Identifies need for intervention by other IDT members.
• Communicates patient and family needs to the IDT effectively.
• Coordinates spiritual care with IDT for continuity and quality of care.
• Maintains updated care plans reflecting problems, goals and interventions.
• Supports nondiscrimination and respects beliefs of families.
