Job summary

Hospice Care Options is seeking a Chaplain to provide spiritual care for patients, families and caregivers. You will assess spiritual needs and develop individualized plans of care as part of the interdisciplinary team, and you may facilitate funeral services.

Bereavement and memorial planning are also part of the role. Responsibilities include coordinating with other caregivers to deliver holistic end-of-life care and supporting families through bereavement.

Qualifications

• Master’s degree in Divinity, Theology or Religion.
• Minimum of one complete unit of Clinical Pastoral Education.
• Reliable transportation and a valid driver’s license.
• Candidate must reside within a 30–40 mile radius of Eastman, GA.

Responsibilities

• Provide spiritual care to patients, families and caregivers.
• Perform spiritual assessments and develop individualized plans of care.
• Coordinate with the interdisciplinary team for holistic end-of-life care.
• Facilitate funeral services when requested by family.
• Provide bereavement care and grief support.
