Job summary

TriHealth Hospice is seeking a Chaplain to provide pastoral care to patients and families in home care settings on the west side of Cincinnati. The role involves independent field-based work, supporting colleagues, and upholding patient-centered care within an interdisciplinary team.

The position requires an MDiv or equivalent, CPE units, and 3–4 years of chaplaincy experience, including hospice. On-call coverage and travel are expected as part of the role.

Qualifications

• Master of Divinity, Theology, or equivalent from ATS-accredited institution.
• Completion of at least three CPE units through ACPE/APC/NACC/NAJC-accredited program.
• Three to four years of professional chaplaincy experience including hospice or healthcare settings.
• Ability to assess and address spiritual and emotional distress in patients and families.

Responsibilities

• Manage a caseload of approximately 50–65 hospice patients and families.
• Provide spiritual assessments, counseling, rituals, and supportive presence.
• Develop and maintain individualized spiritual care plans aligned with hospice philosophy.
• Document patient interactions and care plans in the electronic medical record.
• Respond to spiritual, emotional, existential, and grief-related crises.
• Support during decline, active dying, death, and bereavement.
• Collaborate with physicians, nurses, social workers, aides, and the care team.
• Participate in on-call responsibilities as assigned.

Education

Bachelor's Degree (Required)

Master's Degree of Divinity / Theology (or equivalent) (Preferred)

Clinical Pastoral Education (CPE) – 3 units (Required)

4 units of Clinical Pastoral Education (Preferred)

Certification pursuit commitment within 3 years (APC/NACC/NAJC/ACPE)
