Job summary

Ohio's Hospice seeks a compassionate Hospice Chaplain to provide spiritual and emotional support for patients and families receiving hospice care. The role emphasizes honoring diverse beliefs, building meaningful relationships, and tailoring care to individual spiritual needs.

This PRN, on-call position involves coverage from 5:00 PM to 8:00 AM on varied days, with the hospice team collaborating to ensure holistic care and smooth transitions at end-of-life moments.

Qualifications

• Master of Divinity or equivalent theological degree preferred.
• Ordained or commissioned minister with ecclesiastical endorsement preferred.
• Board certification via APC, BCC-HPCC, NACC, or NAJC preferred.
• CPE: four units preferred; minimum one unit toward completion.

Responsibilities

• Provide spiritual and emotional support to hospice patients and families.
• Establish and maintain compassionate relationships with patients, families, and caregivers.
• Assess spiritual and emotional needs and provide individualized support.
• Provide spiritual care, counseling, presence, prayer, or rituals per patient wishes.
• Support patients and families through end-of-life, grief, and transitions.
• Respect diverse faith traditions and personal values.
• Collaborate with the interdisciplinary hospice team to support holistic care.
• Document services and patient care accurately and timely.
• Travel to patient homes, facilities, and other care settings as needed.
• Participate in on-call coverage during assigned hours.
