Job summary

St. Elizabeth Healthcare is seeking a Hospice Chaplain to provide spiritual care to patients, families, and staff as part of the interdisciplinary team.

You will conduct assessments, lead spiritual interventions, and coordinate with care coordinators to support emotional and spiritual well-being. Qualified candidates have a Bachelor's degree and at least one unit of CPE, with strong communication skills and the ability to work in a collaborative, diverse environment.

Responsibilities

• Conduct spiritual assessment within 5 working days of admission.
• Provide spiritual support and develop a plan of care with the team.
• Document assessments, plans and visits in the patient chart promptly.
• Participate in IDG meetings and coordinate with clergy as requested.
• Educate volunteers and staff on spiritual care practices.
