Job summary

Affinity Hospice Care Inc. is seeking a compassionate Hospice Chaplain to provide in-home spiritual care to patients and families. The role emphasizes collaboration with an interdisciplinary team to assess spiritual needs and deliver tailored support throughout the hospice journey.

The position offers per diem engagement with a pay range of $40–$50 per visit, flexibility in scheduling, and opportunities to participate in quality care and community outreach.

Qualifications

• Graduate of an accredited seminary or school of theology.
• Experience working with death and dying, individuals/family/caregiver.
• Ability to work as a member of an interdisciplinary team.
• Comfort in an interfaith setting.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Assist in development of individualized plan of care through comprehensive assessments.
• Attend patient care conferences as a member of the interdisciplinary team.
