Job summary

Phoenix Home Care and Hospice is seeking a Hospice Chaplain to join our Topeka, KS team. This role includes bereavement coordination and providing spiritual and emotional support to patients, families, and staff in alignment with diverse beliefs.

Responsibilities include spiritual assessments, interprofessional collaboration, and leading funeral or memorial services as requested, along with ongoing professional development.

Qualifications

  • Ordained, commissioned, or credentialed according to a recognized religious group.
  • Completed at least one unit of Clinical Pastoral Education (CPE).
  • A degree in theology, divinity, or equivalent education from an accredited seminary.
  • Comfortable working in interfaith or multicultural environments.
  • Hospice or palliative care experience preferred.
  • Experience or training in grief support and bereavement counseling preferred.
  • Demonstrates deep empathy and a compassionate approach to end-of-life care.

Responsibilities

  • Conduct spiritual assessments within five days of patient admission.
  • Collaborate with the interdisciplinary team to integrate spiritual care into the plan of care.
  • Provide spiritual support to patients and families in alignment with beliefs and preferences.
  • Serve as liaison with community clergy or faith leaders when requested.
  • Offer prayers, services, and rituals for comfort and peace.
  • Maintain accurate clinical documentation (assessments, visit notes).
  • Participate in IDG and QAPI meetings.
  • Support Phoenix staff and participate in bereavement services as needed.
  • Develop and oversee bereavement program and follow-up care plans for families.
  • Coordinate bereavement support for up to 13 months after a patient's death.
  • Lead funeral or memorial services upon request.
  • Pursue ongoing professional education.
