Job summary

Phoenix Home Care and Hospice in Topeka, KS, seeks a Hospice Chaplain to provide spiritual and emotional support to patients and families, and to serve as Bereavement Coordinator. The role works with the care team to integrate spiritual care, conduct assessments, and coordinate bereavement services.

The candidate should be ordained or credentialed, hold a theology degree with CPE, and have interfaith and hospice experience. Bereavement counseling training is preferred.

Qualifications

  • Ordained, commissioned, or credentialed per a recognized religious group.
  • Completed at least one unit of Clinical Pastoral Education (CPE).
  • Degree in theology or divinity from an accredited seminary.
  • Interfaith or multicultural ministry experience encouraged.
  • Hospice or palliative care experience preferred.
  • Bereavement counseling training preferred.
  • Empathy and genuine heart for end-of-life care.

Responsibilities

  • Conduct spiritual assessments within five days of patient admission.
  • Collaborate with the interdisciplinary team to integrate spiritual care into the plan of care.
  • Provide spiritual support aligned with patient beliefs and preferences.
  • Serve as liaison with community clergy or faith leaders when requested.
  • Offer prayer, worship services, and rituals for comfort and peace.
  • Keep accurate, timely clinical documentation, including assessments and visit notes.
  • Participate in IDG and QAPI meetings.
  • Offer spiritual support to Phoenix staff and bereavement services as needed.
  • Develop and oversee the bereavement program with risk assessments and follow-up care.
  • Coordinate and provide bereavement support to families for up to 13 months after death.
  • Lead funeral or memorial services upon request.
  • Continue professional growth through ongoing education, conferences, and self-study.

  • Offer spiritual support to Phoenix staff and participate in bereavement services as needed
  • Develop and oversee the bereavement program, including risk assessments and individualized follow-up care plans for grieving families
  • Coordinate and provide bereavement support to families for up to 13 months following a patient's death, including phone calls, mailings, and support groups
  • Lead funeral or memorial services upon request
  • Continue professional growth through ongoing education, conferences, and self-study
What We're Looking For:
  • Ordained, commissioned, or credentialed according to the standards of a recognized religious group
  • Completed at least one unit of Clinical Pastoral Education (CPE)
  • A degree in theology, divinity, or equivalent education from an accredited seminary
  • Comfortable and experienced working in interfaith or multicultural environments
  • Experience with hospice or palliative care strongly preferred
  • Experience or training in grief support and bereavement counseling preferred
  • A deep sense of empathy and a true heart for those facing end-of-life transitions
