Role and Responsibilities:
- Report directly to the Administrator, Executive Director, or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals and assist in feeding patients when needed.
- Assist the patient in ambulation and with the safe use of mobility aids.
- Perform light housekeeping tasks such as emptying trash and changing linens.
- Assist with treatments as ordered and permissible by regulations.
- Promote patient mental alertness through engagement in activities.
- Provide emotional and psychological support.
- Report changes in patient condition or home situation to the team.
- Demonstrate basic care to caregivers and practice infection control.
- Complete accurate documentation and participate in staff meetings and quality improvement activities.
Requirements:
- Compassionate, kind, and empathetic nature.
- Comfortable providing care in patients' homes.
- Flexible and able to support in crises.
- Certified Nursing Assistant (CNA) license.
- Six months of relevant patient care experience preferred.
- Current driver’s license, automobile insurance, and CPR certification.
