Job summary

Three Oaks Hospice is seeking a Certified Nursing Assistant (CNA) to provide compassionate care to patients in home or facility settings. You will assist with activities of daily living, monitor health status, and report observations to the supervising RN.

Candidate must hold a valid CNA license, state certification, and a current driver’s license with reliable transportation. Hospice or long-term care experience is preferred to support patient comfort and family needs.

Qualifications

• Must have CNA license and be listed in good standing on the state nurse aide registry.
• High School Diploma preferred.
• Must have a valid driver’s license and reliable transportation.
• At least one year experience as Hospice Aide or Nursing Assistant in healthcare settings preferred.

Responsibilities

• Provide personal healthcare and related services to patients in their place of residence; ensure a safe, clean environment under RN supervision.
• Assist patients with activities of daily living and monitor health status; communicate observations to supervising nurse.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY

Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

Qualifications

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
