Job summary

Elevation Hospice of Colorado is actively seeking a Certified Nursing Assistant (CNA) to join our hospice care team serving Parker and surrounding communities. The primary role is to provide personal healthcare and assist with daily living activities under the supervision of a registered nurse, ensuring a safe, comfortable home environment for patients.

Qualified candidates will hold a CNA license, have nurse aide training approved by the state, and at least one year of hospice or long-term care

Qualifications

• Must have CNA license
• Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• High School Diploma preferred
• Must have reliable transportation and valid auto liability insurance
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility

Responsibilities

• Provide personal healthcare and related services to patients in their place of residence
• Assist in maintaining a safe and clean environment
• Work cooperatively with patient and family and share observations with the supervisor (RN)

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
