Job summary

Agape Hospice Care is seeking a compassionate Certified Nursing Assistant (CNA) to join our hospice team serving Frederick and surrounding Colorado communities. You will provide personal healthcare and comfort to patients in their homes, helping maintain dignity and quality of life.

Under the supervision of a registered nurse, you will assist with daily living activities, monitor patient needs, keep environments safe, and communicate observations to the care team.

Qualifications

• High School Diploma Preferred.
• Has CNA license in good standing.
• Must have current driver’s license.
• One year or more of hospice or nursing assistant experience preferred.

Responsibilities

• Provide personal healthcare and comfort to patients in their homes under RN supervision.
• Assist with daily living activities and monitor patient needs.
• Maintain a safe, clean environment and report observations to the care team.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY:

Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver's
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
