Job summary

Residential Home Health and Hospice is seeking a dedicated Hospice CNA to provide compassionate in-home care to patients in the Oak Brook, IL area. You will join an interdisciplinary team focused on comfort, dignity, and quality of life while delivering hands-on support in patients' homes.

Qualifications

• Active CNA certificate in the state you work.
• High school diploma or equivalent.
• Minimum one year healthcare experience; hospice experience preferred.
• Valid driver’s license and ability to travel to patient locations ~20% of the day.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Assist with activities of daily living (ADLs) and observe changes in condition.
• Reinforce care plans and provide emotional support to patients and caregivers.
• Document care in the EMR and respond promptly to patient needs.
• Work independently, but collaborate with the interdisciplinary team; travel within assigned territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
