Residential Home Health and Residential Hospice in Northbrook, IL is seeking a Certified Nursing Assistant to provide in-home care to patients in surrounding communities.

This 32-hour-per-week, light full-time CNA role emphasizes compassionate, patient-centered care and collaboration with an interdisciplinary team. A valid CNA license in the state and a valid drivers license are required.

Qualifications

• Active CNA certificate valid in the state you work.
• High School Diploma or equivalent required.
• Minimum one year healthcare experience.
• Must have a valid drivers license and be able to travel to patient homes (about 20% of workday).

Responsibilities

• Deliver compassionate, patient-centered care in patients homes, coordinating with the interdisciplinary team.
• Assist with activities of daily living (ADLs) and monitor changes in condition.
• Educate and support patients and caregivers on care plans and routines.
• Document care and communications accurately in the EMR.
• Work independently while staying connected with the care team.
• Travel to patient homes within the assigned territory.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
