Job summary

Heritage Hospice is resuming services in the Albuquerque community. We are seeking a compassionate PRN Hospice CNA to join our interdisciplinary care team, providing personal care, comfort, and supportive services to patients and families.

This role offers flexible scheduling and a meaningful impact within a devoted hospice team. The CNA will deliver hands‑on care, maintain safe home environments, and communicate changes to the RN Case Manager while traveling to patient homes and facilities

Qualifications

• Current New Mexico CNA certification.
• Hospice or home health experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently in home settings with professionalism.

Responsibilities

• Provide hands‑on personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL support.
• Assist with maintaining a safe, clean, and comfortable environment in the home or facility setting.
• Observe and report changes in the patient’s condition to the RN Case Manager.
• Offer emotional support to patients and families with sensitivity and respect.
• Document care accurately and in accordance with regulatory and organizational standards.
• Participate in team communication to ensure coordinated, high‑quality care.
• Travel to patient homes, facilities, and communities within assigned coverage area.

Key Responsibilities

• Provide hands‑on personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL support

• Offer emotional support to patients and families with sensitivity and respect

Qualifications

• Current New Mexico CNA certification
• Prefer at least six months direct patient care experience; hospice or home health experience preferred
• Reliable transportation and valid driver’s license
• Ability to work independently in home settings with professionalism and compassion
• Strong communication, time‑management, and interpersonal skills
• Comfort supporting patients and families during end‑of‑life transitions
