Job summary

Del Corazon Hospice is seeking a compassionate Hospice Certified Nursing Assistant (CNA) to care for patients primarily in Albuquerque, NM. The role requires traveling daily to SNFs, Assisted Living facilities, and private homes within the service area, providing hands-on support and dignity.

As a team member you will work with nurses, social workers, and chaplains to deliver high-quality end-of-life care. A NM CNA license is required and reliability with transportation is essential.

Qualifications

• Current New Mexico CNA license is required.
• Hospice/home health experience preferred.
• Valid driver's license and reliable transportation.
• Ability to travel daily within service area.
• Excellent communication and bedside manner.
• Ability to work independently and in a team.
• Background check and drug screening required.

Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)
• Observe and report changes in patient condition to the clinical team
• Support families with education and comfort measures
• Maintain accurate documentation per hospice and regulatory guidelines
• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team
• Maintain patient dignity, privacy, and comfort at all times
• Travel throughout the service area daily

Position Overview

We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Albuquerque, providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients’ homes. This role requires compassionate hands-on care, excellent communication, and a heart for hospice.

Key Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)
