Job summary

Dartmouth Health is seeking a Nursing professional to coordinate and manage a home-based hospice caseload under the Clinical Manager. You will assess, plan, implement, and evaluate care with the interdisciplinary team, supervise Home Health Aides, and ensure timely, quality patient outcomes in Vermont and New Hampshire.

Requirements include an active RN license in VT/NH or a compact license, graduation from an accredited nursing program, and preferred hospice/homecare experience.

Qualifications

• Must be a licensed RN in VT and NH or hold a compact license.
• Graduate of an accredited school of nursing.

Responsibilities

• Provide full range of professional nursing care to patients in their homes under physician direction.
• Regularly reevaluate patient needs and inform team of changes in condition.
• Initiate and revise the plan of treatment as needed.
• Supervise Home Health Aides and educate in-home care.
• Assess patient/family needs to coordinate hospice care plans.
• Represent Dartmouth Health positively in community activities.
• Maintain continuity of care by exchanging information with IDT and other agencies.
