The Hospice Case Manager, RN is a part-time role based in Litchfield, MN, responsible for providing direct nursing care and case management to a diverse client population in home and community settings.

The position offers flexible hours (40 hours per pay period, M-F, 8a-4:30p) with an on-call rotation, and pay ranges from $39.00 to $45.00 per hour based on experience and qualifications.

Minimum requirements include a nursing degree, RN licensure in Minnesota, and at least one year of post-acute care experience. Preferred qualifications include a BSN or CHPN credential and management experience.
