RN Case Manager - Full-Time

Schedule: Full-Time, M-F 8am - 5pm (can be flexible).Location: Northwestern Suburbs of Minneapolis, MN. Relatively large service area including NW Metro suburbs such as Fridley, Brooklyn Park, Maple Grove, Medina, Delano, St. Michael/Albertville, Big Lake, Elk River, Oak Grove, Anoka, Dayton, MN. (Mileage reimbursement will be provided at $0.70/mile!)

Pay rate:Up to $51.67 depending on experience!

*This is a temporary assignment that is expected to be temp-to-hire!

Position ResponsibilitiesInclude:

  • Perform assessments/follow up assessments and procedures, match employee skills with the clients' needs and develop working relationships with discharge planners and social service workers.
  • Obtain physicians plans of treatment and initiate execution, and provide services ordered by the physician as indicated in the care plan.
  • Counsel and educate the patient/family in meeting healthcare related needs, and effectively communicate with members of the interdisciplinary healthcare team.
  • Complete timely and accurate documentation/clinical notes including addressing the patient's progress.
  • Supervise paraprofessional and licensed professional team members (LPNs).Secure equipment and supplies and coordinate placement for use in patient's homes, and refer patients to other services as needed.
  • Participate in Quality and Performance Improvement (QAPI) program, along with the Office's Emergency Preparedness program as requested.
  • Follow accepted standards of nursing practice and use clinical decision making to efficiently and effectively manage an assigned patient population.
  • Access personal health information (PHI) necessary to fulfill the above duties and responsibilities.
  • All other duties as assigned.

Required Skills/Qualifications:

  • An active RN license in the states of MN.
  • Two years of current RN experience is preferred.
  • One year of RN Case Management experience is preferred.
  • Must have reliable transportation for patient visits. (Opportunity for company vehicle available!)
