Job description

Hospice Case Manager, RN | Part-Time | Litchfield, MN Territory.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of clients in accordance with agency standards and policies. This position performs skilled nursing functions in home and community settings, serving the nursing needs of patients, families, and groups throughout the lifespan.

Opportunity Details:

  • Schedule: Part-Time, 40 hours per pay period, with shifts between 8 a.m. and 4:30 p.m., Monday through Friday. Flexible on-call rotation.
  • Pay Range: $39.00 - $45.00 per hour, based on experience, qualifications, certifications, licenses, and education.

Essential Job Responsibilities:

  • Deliver skilled services efficiently to meet patient and family needs.
  • Conduct initial and ongoing comprehensive assessments of patients and families.
  • Collaborate with physicians, healthcare professionals, patients, and families to develop and implement coordinated care plans.
  • Develop and evaluate plans of care with measurable goals, updating as needed.
  • Ensure accurate communication of physician orders to staff.
  • Initiate case conferences to discuss patient progress and care plans.
  • Promote safety for clients and staff.
  • Perform other duties as assigned.

Minimum Required Qualifications:

  • Graduate of an accredited nursing program.
  • At least 1 year of nursing experience in a post-acute care setting.
  • Current RN license in the state of practice, maintained actively.
  • Effective verbal and written communication skills.
  • Proficiency with Microsoft Office Suite.
  • Knowledge of safety regulations, HIPAA, Resident’s Bill of Rights, and other relevant policies.

Preferred Qualifications:

  • BSN or RN with CHPN credential.
  • Previous management experience.
