Job summary

Residential Home Health and Hospice is seeking a Hospice Transitional Nurse Liaison to partner with field staff and leaders in delivering top-tier hospice care. The role involves comprehensive patient assessments, coordinating care, and educating patients and facilities about services.

Ideal candidates hold an RN with an Associate degree, at least two years in nursing (hospice preferred), and experience with EPIC and HCHB. On-site position with equal opportunity employment.

Qualifications

• Associate degree from an accredited School of Nursing.
• Minimum 2 years of RN experience including acute care or oncology; hospice experience preferred.
• Experience with Microsoft Office Suite, EPIC, and HCHB preferred.

Responsibilities

• Perform initial and ongoing patient/family hospice assessments.
• Collect diagnosis, medical history, and care information; coordinate start-of-care.
• Explain services and coordinate start-of-care visits; order DME, supplies, and meds.
• Deliver community education for facilities and hospitals; educate patients at bedside.
