Job summary

Well Care Health is seeking a Hospice Clinical Manager to lead the clinical interdisciplinary team in Advance, NC. The role oversees daily operations, ensures regulatory compliance, and mentors nursing and support staff to deliver high-quality patient care.

The manager will coordinate care plans, supervise documentation and performance, and collaborate with leadership to optimize services and outcomes within the hospice program.

Qualifications

• Graduate of NLN accredited school of nursing with current RN license.
• Bachelor’s degree preferred.
• Strong leadership and interpersonal skills required.
• Experience supervising clinical staff in hospice or similar setting preferred.

Responsibilities

• Oversees daily operations of the hospice clinical team and serves as a resource to interdisciplinary members.
• Directs implementation of care processes compliant with regulations and quality standards.
• Supervises documentation, care plans, and compliance with agency and accreditation requirements.
• Provides mentoring, coaching, and education to clinical staff.
• Participates in IDG meetings and collaborates with Hospice Medical Director on patient goals.
• Supports budgeting, staffing, and policy development to improve service delivery.
