Amedisys is seeking a hospice liaison to develop and manage relationships with hospitals, nursing homes, and physician offices in the Greater Boston area. You will educate healthcare professionals and families about home care services and coordinate admissions with the care team.

The role emphasizes site visits, lead generation, and meeting admissions goals while navigating patient needs and payer requirements in a dynamic healthcare environment.

Qualifications

• Minimum of 1 year in healthcare-related business development/outreach.
• Experience in outside sales or account development within healthcare settings is preferred.
• Reliable transportation and valid driver's license.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with hospitals, nursing homes, and physician offices to generate leads.
• Coordinates patient admissions with the team and collects required documentation for referral to admission.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Maintains CRM records of patient interactions, referrals, and daily sales calls.
• Achieves admissions, product, and payor-mix goals; meets account targets and quotas.
