Job summary

Amedisys is seeking a hospice liaison to educate health care professionals, patients and families about home care services and to develop referrals within the Green Bay/Appleton territory.

This field-based role travels 50%+ within the territory and offers competitive pay, bonus potential, choice of medical plans, paid time off, 401(k) with match, wellness rewards and family support programs.

Qualifications

• At least one year of experience in healthcare business development/outreach.
• Bachelors degree preferred but not required.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with referral sources to generate leads.
• Coordinates admissions with the team and manages documentation for referrals.
• Keeps accurate records of patient interactions and referrals in CRM.
• Attends community events to promote Amedisys.
