Job summary

Lake Sunapee VNA and Hospice seeks an experienced Nurse Practitioner for its Hospice program on a per diem basis to deliver direct patient care visits, manage symptoms, and conduct Hospice Face to Face assessments. The role includes an on-call rotation and travel to patient homes for care visits.

The ideal candidate has NH NP licensure, at least two years of healthcare experience, and familiarity with hospice or end-of-life care.

Qualifications

• Current NH Nurse Practitioner (NP) license.
• Minimum 2 years of health care experience.
• Hospice, Palliative or end-of-life care experience is preferred.

Responsibilities

• Provide direct patient care visits for routine provider visits and symptom management in Hospice.
• Participate in Hospice Face to Face visits and on-call rotation as required.
