Lake Sunapee Region VNA & Hospice seeks an experienced Nurse Practitioner to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. On-call rotation required.

Qualifications

• Experience in hospice and/or end-of-life care with sympathetic management.
• Strong communication and ability to collaborate with an interdisciplinary team.
• Comfort educating and guiding patients, families, facilities in end-of-life discussions.

Responsibilities

• Provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face.
• On-call rotation required.
