Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. An on-call rotation is required.

Qualifications

• Current NH Nurse Practitioner license.
• Minimum 2 years healthcare experience.
• Hospice, palliative, or end-of-life care experience preferred.

Responsibilities

• Provide direct patient care visits for routine provider visits and symptom management.
• Conduct Hospice Face to Face visits as required.
• Participate in on-call rotation and collaborate with interdisciplinary team.
