Job summary

Lake Sunapee Region VNA & Hospice is seeking an experienced Nurse Practitioner to join our Hospice program in a per diem capacity, delivering direct patient care visits for routine provider visits and symptom management, including Hospice Face to Face assessments.

The role requires participation in on-call rotation and collaboration with an interdisciplinary team to support patients and families in end-of-life care at home. Strong communication and NH NP licensure are expected.

Qualifications

• Experience in hospice/end-of-life care is preferred.
• Strong communication and ability to collaborate with an interdisciplinary team.
• Comfortable educating patients, families, facilities and communities on end-of-life discussions.

Responsibilities

• Provide direct patient care visits for hospice patients.

Qualifications:

• Current NH Nurse Practitioner (NP) license
• Minimum 2 years of health care experience required
• Hospice, Palliative or end-of-life care experience is preferred
