Job summary

Ovation Hospice in Utah is seeking a Patient Advocate (Hospice Liaison) to connect families with compassionate end-of-life care. In a field-based role, you will build trusted relationships with physicians, hospital teams, SNFs, and case managers, guiding referrals from ground up and ensuring a smooth patient hand-off.

You will educate referral sources about Ovation’s capabilities and criteria, discuss hospice benefits with families, and drive admissions growth while maintaining meticulous CRM

• Demonstrated ability to build trust quickly in clinical or community settings.
• Strong self-direction with ability to plan and manage a territory.
• Resilience to rebound from slow weeks or missed referrals.
• Driven to meet or exceed referral and patient conversion goals.

Responsibilities

• Build and manage a referral territory with physicians, SNF staff, and care teams.
• Educate referral sources on Ovation’s capabilities and patient criteria.
• Explain hospice benefits to families and help them understand options during emotional times.
• Generate qualified referrals and convert them to patients in care.
• Maintain accurate CRM activity: visits, contacts, and pipeline.
• Coordinate with Administrator, clinical team, and intake to ensure smooth hand-offs.
