Job summary

Ovation Hospice is seeking a Patient Advocate (Hospice Liaison) in Utah. This field-based role focuses on building trusted relationships with physicians, hospital discharge teams, SNFs, and families to connect them with compassionate end-of-life care.

You will develop a referral territory, manage visits, and collaborate with clinical teams. This cohort-based hiring across all markets offers a shared start date, onboarding, and compensation structure.

• Self-directed with ability to plan week, territory, and follow up independently.

Responsibilities

• Build and manage a referral territory by cultivating relationships with physicians, hospital case managers, SNF social workers, and healthcare partners.
• Educate referral sources on Ovation's capabilities, patient criteria, and timely hospice referrals.
• Meet with families to explain hospice benefits and options during emotional times.
• Drive admissions growth by generating qualified referrals and converting them to patients.
• Maintain accurate activity tracking in CRM—logging visits, contacts, and pipeline.
• Collaborate with Administrator, clinical team, and intake coordinators for seamless hand-offs and patient experience.
• Represent Ovation in the community with professionalism and genuine care.
