Job summary

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Collaborate with interdisciplinary hospice team and attend weekly meetings.
• RN Case manager will interact with physicians and hospice medical director as needed to manage symptoms.
• Manage comprehensive patient assessments, care planning and supervision of care.
• Support Hospice Aides and LPN/LVNs as needed.
