Job summary

Enhabit Home Health & Hospice in Georgia seeks an experienced registered nurse to supervise hospice services and ensure compliance with care standards. The role coordinates care between physicians, patients, families, and field staff to deliver timely, high-quality home health services.

The ideal candidate holds an RN license, CPR, a valid driver's license, and experience with Homecare Homebase; you will lead care coordination, scheduling, and regulatory adherence in a collaborative team

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• CPR certification and valid state driver\'s license.

Responsibilities

• Coordinate hospice services according to standards and procedures.
• Review and approve patient information during start of care, recertification, or evaluations.
• Facilitate relationships among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and ensure care coordination and continuity.
