Job summary

Advocate Health’s Atrium Health Hospice Cabarrus - Patient Access department in North Carolina is seeking a full-time RN for clinical administrative support and care coordination. The role involves liaison with physician practices, hospital case management, and other referral sources, with 40 hours per week and M-F 10-hour shifts.

Weekend coverage may be required; shifts could move to 12 hours. The position requires RN licensure, BSN preferred, and a minimum of 1 year in a related field.

Qualifications

• Graduate from an accredited School of Nursing.
• RN license or temporary RN Practitioner license in the state of work.
• 1 year experience in a related field.

Responsibilities

• Provides clinical administrative support and liaison with physician practices and hospital case management.
• Analyzes data to prioritize multi-issue situations to support timely actions.
• Identifies barriers to resolution and follows up with leadership.
• Reviews and supports development of patient-specific care plans and communicates with involved parties.
• Supports clinical staff in implementation of the plan and care coordination.
• Evaluates progress toward outcomes through documentation review and team meetings.
• Supports billing compliance and timely CSC workflow per payer requirements.
• Delegates elements of care to appropriate healthcare workers per policy.
• Collaborates with patients, families, and others in clinical practice.
• Uses resources to provide safe, effective, and financially responsible services.
• Participates in committees and governance activities.
