Job summary

Advocate Health Hospice Cabarrus is seeking a clinically experienced RN to join the Patient Access team in Cabarrus County. The role focuses on coordinating care across physicians, hospital case management, and referral sources to ensure timely services and patient satisfaction.

The position requires an active RN license, BLS, and at least one year of related experience. It offers full-time hours, 40 per week, with potential for shift adjustments within M-F and weekend coverage as needed, and

Qualifications

• RN license required in the practice state.
• BSN degree preferred; BLS certification required.
• Minimum 1 year of related field experience.

Responsibilities

• Provides clinical administrative support, liaises with physician practices, hospital case management and referral sources.
• Analyzes data to prioritize multi-issue situations for patient health and safety actions.
• Identifies barriers to resolution and follows up with leadership to address timely outcomes.
• Reviews and supports patient care plans to ensure quality and satisfaction.
• Supports clinical staff in care delivery and coordination of services.
• Evaluates progress toward outcomes via documentation review and team meetings.
• Supports billing compliance and timely CSC workflow to maximize appropriate reimbursement.
• Delegates care elements to appropriate healthcare workers per policy.
• Collaborates with patients, families, and providers in clinical practice.
• Uses resources to plan safe, effective, and financially responsible clinical services.
• Participates in governance and facility decision-making.
