Job summary

Advocate Health is seeking an experienced Registered Nurse for home health in Cabarrus County, NC. This full-time, salaried role offers 40 hours per week with a sign-on bonus eligible up to $15,000 and a referral bonus of $7,500 for internal teammates.

You will manage assigned patient caseloads, document care, and collaborate with MD orders. Required are a valid RN license, BSN preferred, and BLS per policy; you will work with a multidisciplinary team to ensure high-quality outcomes and patient

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• BLS required per policy guidelines.
• Current RN license in the state of work or temporary license as a Registered Nurse Practitioner.
• Additional education, training, certifications, or experience may be required within the department.

Responsibilities

• Manage daily patient caseload, ensuring timely care and documentation per plan of care and MD orders.
• Collect data from assessments and EMR to evaluate patient health and needs.
• Analyze data to identify high-risk patients and coordinate interventions to improve outcomes.
• Prioritize care based on patient health, safety, and timely service delivery.
• Identify barriers to resolution and coordinate with leadership to address them.
• Develop and oversee a plan of care and communicate with involved parties.
• Coordinate care with a multidisciplinary team and supervise LPNs and HHAs.
• Perform ordered treatments, therapies, and procedures per policy and regulations.
• Evaluate progress toward patient-centered goals.
