Job summary

Advocate Health seeks a part-time RN for community palliative care and clinical administrative support. The role interfaces with physician practices and hospital case management to ensure timely services. Mon-Fri, 8:30am-5pm, with pay range of $38.20–$57.30 per hour and comprehensive benefits.

Responsibilities include care plan development, coordination, billing compliance, and collaboration with care teams to promote safe, effective outcomes.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• RN license in the state of employment or equivalent.
• BLS certification required.

Responsibilities

• Provides clinical administrative support and liaison with physician practices and case management.
• Analyzes data to prioritize multi-issue situations affecting patient health and safety.
• Reviews and supports development of patient care plans and communicates with involved parties.
• Supports clinical staff in plan implementation and coordination of care delivery.
