Job summary

Friendship Village Chesterfield is seeking a Hospice CNA to provide compassionate personal care under the direction of a registered nurse. The role emphasizes honesty, integrity, and alignment with our hospitality-focused care environment.

The position requires completion of a state-approved geriatric nursing assistant program, state registry registration, and CPR certification. A valid driver's license with an insured vehicle is also required to support patient transport and home visits.

Qualifications

• Must have a valid driver’s license with an insured vehicle.
• Completion of an state-approved geriatric nursing assistant training program and State registry registration are required.
• CPR certification is mandatory.

Responsibilities

• Provides personal care (bathing, dressing, toileting, feeding) per the Hospice Aide Plan of Care.
• Documents care provided according to agency policy.
• Reports deviations from the Plan of Care to the Case Manager.
• Uses durable medical equipment safely.
• Assists with ambulation, transfers, and range of motion as documented.
• Take vital signs per the Hospice Aide Plan of Care.
• Communicates changes in resident condition or family interactions promptly.
• Participates in QAPI activities and IDG meetings as appropriate.
