Job summary

Kindful Health in Plano, TX seeks a compassionate AIDE to work with the RN Case Manager to deliver best-in-class hospice care to a reasonably sized patient load. You will participate in IDT updates every two weeks and coordinate with care team leaders to maintain high-quality service.

The role emphasizes regulatory compliance, effective communication with patients and families, and collaboration across the care team to ensure exceptional CAHPS scores.

• Collaborate with RN Case Manager, Clinical Director, Medical Director and team to provide concierge hospice care.
