Job summary

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• NAC certification.
• CPR certification (hands-on preferred).
• High school diploma or GED.
• 18 years of age or older.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Provide patient and family with respectful communication and confidentiality.
• Assist in dressing, grooming, feeding, and simple transfers.
• Report pain or changes in condition to the RN Case Manager.
• Maintain patient living area clean and orderly; document care provided.
