Job summary

Sarah Bush is seeking a Bereavement Counselor (Hospice) for part-time hours to provide grief assessment, counseling, and psycho-social support to patients and caregivers in the Hospice service area.

You will participate in grief support programs, train volunteers, document care activities, and collaborate with the interdisciplinary team to support survivors.

Bachelor's degree required; experience in bereavement programs encouraged; daytime hours available with flexible scheduling.

Qualifications

• Bachelor's degree required.
• Experience in bereavement programs or hospice care preferred.
• Strong communication and counseling skills.

Responsibilities

• Assists in ongoing development of the bereavement program to improve care/services and outcomes.
• Assists in training and supervision of Hospice volunteers.
• Assists with implementation of all components of the Bereavement Program.
• Completes documentation accurately, thoroughly and timely per Hospice policy.
• Facilitates bereavement support groups and other educational programs for community members.
• Functions as a member of the Interdisciplinary team and attends team meetings to review and revise the plan of care.
• Maintains a caseload of survivors to provide bereavement follow-up consistent with Hospice policies.
• Provides bereavement counseling, support and education to Hospice patients, caregivers, LTCFs, staff, and community members.
