Job summary

Sarah Bush Lincoln in Oklahoma is seeking a Bereavement Counselor for part-time hours to provide grief assessment and psycho-social support to patients and caregivers, and to participate in community grief education within the Hospice service area.

Responsibilities include developing and improving the bereavement program, training Hospice volunteers, documenting per policy, and facilitating support groups while collaborating with the interdisciplinary team to review care plans and maintain

Responsibilities

• Assists in the ongoing development of the bereavement program to improve care/services and their outcomes.
• Assists in training and supervision of Hospice volunteers.
• Assists with the implementation of all components of the Bereavement Program.
• Completes documentation accurately, thoroughly and timely per Hospice policy.
• Facilitates bereavement support groups and other educational programs for community members.
• Functions as a member of the Interdisciplinary team, attends Interdisciplinary team meetings to review and revise the plan of care and to report on the status of survivors.
• Maintains a caseload of survivors to provide bereavement follow-up consistent with policies of the Hospice program.
• Provides bereavement counseling, support and education to Hospice patients, caregivers, LTCFs, staff, and community members.
