Job summary

Valor-Hospice is seeking a Hospice Aide/Nursing Assistant to provide direct patient care in the patient’s home under the direction of a registered nurse. The role supports transfers, ambulation, reporting changes, and documentation to ensure continuity of care while maintaining infection control and a clean, safe environment.

The ideal candidate will have a High School Diploma (preferred), completion of an approved aide program or competency evaluation, and a current driver’s license.

  • Demonstrates care for ill and elderly patients.
  • Successful completion of Aide Competency Evaluation per §418.76.

Responsibilities

Job description

Primary function is to provide personal healthcare and related services to the patient in their place of residence; toassist in providing a safe and clean environment; work cooperatively with the patient, and family; and share observations andproblems with the supervisor. Works under the direction and supervision of a registered nurse.Essential Functions:Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care.Provides necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy.Provides necessary skill to appropriately report changes and document pertinent information and care rendered topatient to ensure continuity of care.Practices accepted infection control principles. Provide a clean, safe, and comfortable environment.Promotes positive, supportive, respectful communication to patient/family and other employees.Provides an environment which promotes respect for patient, privacy, and property.Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake.Contributes to the management and efficient operation of the Hospice and demonstrate effective time managementskills.Provides skills necessary to perform treatments and procedures according to the HospiceEducation: High School Diploma Preferred. Has successfully completed one of the following: A) Trainingprogram/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations; B) Competencyevaluation program that meets §418.76( c); C) Nurse aide training/competency evaluation program approved bythe state and listed in good standing on the state nurse aide registry; D) State Licensure program that meetsrequirements of §418.76(b).Licensure: Must have a current driver’s license.Experience: At least one year experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, homehealth agency, hospice or long-term care facility or completion of Certified Nursing Assistant or Certified HomeHealth Aide Program.Skills: Must be able to read, write, and verbally report clinical information to patients, representatives, and care givers,as well as to other hospice staff. Demonstrates interest in the welfare of the ill and elderly. Successful completionof the Aide Competency Evaluation Skills Checklist that meets §418.76 and written Skills Test.Transportation: Reliable transportation and valid auto liability insurance.
